I hear a rumor that if you refine the first wep before make the other one then can fail, repeat just a rumor i don't test it, i make 3 weps for my char with 100% without refine them.
Refine it in 2nd version (1st cannot be refined). All refinements are passing by to next upgrade (you don't lose ref while upgrading weapon). This way even if you fail you gonna save your money you would pay for jade statues. I did the same - lucky me I'm sitting with 3 YD weapons +3,+6,+7 - ran out of BE's. Too bad it can't be put in storage.
There is an entire guide for this in the guide section. The weapon can fail upgrade when you refine it.
